How do you write a query letter to a literary agent?

Ok, so I’ve been looking around, and studying them, but I’m still not sure about sending query letters to a literary agent. What all do they need? How do you address the people if you don’t know who is going to read it? If possible, could someone give me the link for an example query letter too?
